On this page you will find all the paperwork you need to enroll in Autumn Leaf Academy.
Although you will need to obtain your child's shot record and physical (Blue and Yellow) forms from your doctors office.
All children attending Autumn Leaf Academy participate in the Federal Child Care Food Program.

Please fill out either the Spanish or English version of the food program paperwork, as well as the childcare enrollment/information form.
You will need to print out four (4) forms (6 pages total) for every child you are enrolling.
